Registered Nurse Case Manager

We are offering an $8,000 sign-on bonus for the position of Registered Nurse Case Manager. In this role, you will provide intermittent nursing services to patients in their homes in collaboration with their physicians and according to agency policies and procedures.

Requirements:

• Graduate of an NLN-approved school of nursing
• One year of recent medical/surgical nursing experience
• Possession of a current license to practice as a Registered Nurse in the State of Connecticut

Key Responsibilities:

1. Admitting and providing intermittent nursing services to patients
2. Documenting all services provided in the clinical record
3. Managing work assignments in an effective and efficient manner
4. Establishing and evaluating the plan of care for the Home Health Aide
5. Developing and maintaining professional skills and credentials
