Role and ResponsibilitiesnnA qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home, following physicians orders.nnEssential FunctionsnnEvaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures.nnPrepare an assessment of the patients condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.nnRecommend modification of plan of care to physician, as needed.nnDocument clinical notes on the date of the visit into medical record and keep all records and summaries up-to-date.nnTreat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence.nnInstruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment (canes, walkers, wheelchairs, etc.).nnInclude and educate the patient and caregiver(s) regarding treatment plan and goals.nnAttend physical therapy staff meetings and in-service presentations.nnParticipate in case conferences.nnParticipate in the development and periodic revision of the physicians Plan of Treatment.nnAssure appropriate safety practices and maintenance of equipment.nnOther duties as assigned.nnThis job description reflects managements assignment of essential functions; and nothing in this herein restricts managements right to assign or reassign duties and responsibilities to this job at any time.
