The Health Resource Manager is responsible for providing case management services for SVMC sub acute patients. $5,000.00 sign on retention bonus for qualified candidatennRequirements:nnGraduation from an accredited school of nursing and a Bachelor’s of Science of Nursing preferred;applicants holding a valid RN license and currently matriculated into a BSN program may be considered.nnPrior experience in utilization review, discharge planning and/or case management preferred.nn3-5years of recent clinical experience required.nnVTNursing License required by date of employment.nnCase Management Certification required within 2 years ofemploymentnnThe case manager is responsible for coordinating a patient-centered plan of care to meet individual needs by:nnCompleting intake interviews to identify a holistic plan of care; facilitating multiple aspects of care coordination with the patient, family/caregiver, providers and other community resources prior to dischargennDeveloping a patient transition plan, providing patient/caregiver education and proactively advocating for patient needs and goals across the interdisciplinary team to ensure an approach that is sensitive to each specific patient and family’s needsnnAssisting providers in determining patient’s level of care in accordance with InterQual criteriannActively seeking out collaboration with providers to improve overall quality and efficiency of carennArea of Interest:Nursing;nnWork Status:days, variable hours;nnJob ID:5459nnDartmouth Health is an Affirmative Action and Equal Opportunity Employer.
