PHA is proud to post this opening on behalf of their member organization, WellSpan Health.nnWellSpan is hiring Home Health Physical TherapistsnnFull time (80 hours biweekly)nMonday - Friday 8:00am - 4:30pm; Flexible schedule optionsnThe selected candidate will work approximately every 6th-8th weekendnImmediate needs in York, Lancaster and Adams countiesnn*Sign On Bonus *nnGeneral SummarynnProvides physical therapy services at an entry to intermediate skills level to individual patients according to the principles and practices of physical therapy and works collaboratively with interdisciplinary treatment team.nnResponsibilities for Internal CandidatesnnDuties and ResponsibilitiesnnEssential Functions:nnEvaluates patient using diagnostic and prognostic muscle, nerve, joint and functional ability tests.nPlans individualized physical therapy program for each patient including exercises, therapeutic modalities and procedures.nDirects and supervises patient in active, active assisted, passive exercises, muscle re-education, and functional training using appropriate equipment as needed.nProvides instructions in therapeutic procedures to be continued by the patient.nCoordinates physical therapy activities in collaboration with the other members of the interdisciplinary team to facilitate achievement of goals established.nConducts or participates in training medical and nursing personnel in physical therapy techniques and objectives.nUtilizes safe patient handling techniques for all patient transfers, lifts and positioning.nProvides supervision of PTAs and Rehab Aides as required by PA practice act.nnCommon Expectations:nnDemonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).nMaintains patient record, as required by profession, with accurate documentation to support medical necessity and functional abilities.nEnsures the process design of established policies/procedures, objectives, quality management, safety, environment and infection control standards are utilized consistently.nEnhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.nActs as a role model in Red Rules compliance with a focus on hand hygiene and patient identifiers.nCollaborates with other team members to create a culture of patient centeredness and clinical excellence.nUtilizes SBAR as a standard method of communication in all aspects of patient care.nProvides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.nnRequired for All Jobs:nnPerforms other related duties as identified.nWellSpan Health has adopted and implemented a compliance program to support WellSpan's values and standards for professionalism, integrity, and ethics.
