Physical Therapist - Home Health
Duties:
- Effective patient care using approved treatment techniques and procedures. Evaluation of patient’s condition, functioning level and treatment goals and keeping current records.
- After evaluating the patient, the therapist is responsible for planning administering and supervising the PTAs and HHAs in accordance with the treatment plan.
- Administers appropriate evaluations to assess patient’s level of function and develops therapy goals. Develops treatment plan for each patient and performs appropriate individual treatment of assigned patients. Documents patient’s treatment in medical chart daily.
- Will provide therapy services as outlined in the Plan of Care.
- Assists patient and family with development and review of home program, evaluation and review of adaptive equipment needed in home setting and evaluation of onsite home setting when necessary.

- Proficiency in using electronic medical record systems
- Excellent communication and interpersonal skills
- Ability to work independently and make sound clinical judgments
