Job Title: LPN - Part Time (24 hours)
Job Specialty: Long Term Care (LTC)

Shift: Part Time, 3x8 hours; 3-8 one week and 2x8-hour shifts the following week, Every other weekend (Friday & Saturday night)

Must-Have: Experience in providing skilled nursing care to geriatric patients
Job Description:
- Demonstrate the knowledge and skills necessary to provide appropriate care to the age of the patients served.
- Possess the ability to assess data reflective of the patient's status and interpret information needed to identify each patient's requirements relative to age-specific needs.
- Provide both direct and indirect skilled nursing care for assigned residents.
- Follow defined policies and procedures under the supervision of a Registered Nurse.
- Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
- Maintain a current level of knowledge and clinical expertise through continuing education and in-service programs.
- Collect data relevant to the resident's physical and psychosocial condition and communicate findings to the RN and other members of the Interdisciplinary Team.
- Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of care, reflecting nursing diagnosis and interventions to achieve desired outcomes.
- Recommend revisions to the plan of care, after evaluating resident outcomes, and indicate the need for referrals to ensure comprehensive care.
- Collaborate with residents, families, and the Interdisciplinary Team within the scope of nursing practice to ensure customer satisfaction.
- Maintain current knowledge and competency in nursing practice and participate in staff mentoring/orientation.
- Consider factors related to safety, cost, and impact when planning and delivering care under the RN's direction.
- Demonstrate knowledge and competency in professional practice, with occupational exposure to blood-borne pathogens anticipated.
