Job Description:
- Provide physical therapy services to patients in a rehabilitation setting
- Collaborate with interdisciplinary team members to develop and implement patient care plans
- Conduct patient evaluations and assessments to determine treatment goals
- Document patient progress and treatment outcomes accurately
- Adhere to all state and facility regulations regarding physical therapy practices
- Participate in ongoing education and training to enhance skills and knowledge
- New Grads are welcome to apply, but must demonstrate a willingness to learn and adapt to the SNF environment
- Ensure compliance with all credentialing requirements prior to starting the assignment.
