Job Description
Job Title: Physical Therapist (PT) - Home Health
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day, 5x8-Hour (08:00 - 16:30)
Guaranteed Hours: Facility can call off up to one shift per pay period.
Experience: 1 year of skilled home health experience required; First-timers not accepted.
License: Pending license accepted; confirm with the hiring manager.
Certifications: BCLS required.
Must-Have: Reliable transportation required; Oasis experience preferred but not required.
Job Description:
- Provide home health physical therapy services in Medford, OR, and surrounding areas.
- Manage patient load with a standard of 30 productivity points, visiting up to 6 patients per day.
- Strict adherence to a 1:1 patient ratio.
- Engage in 40 hours of non-billable orientation.
- Expected to complete modules pre-start and annually; average of 16 hours.
- Maintain accurate patient records using EPIC charting system.
- Ensure adherence to specific shift requirements and cannot accommodate 4x10s or 3x12s shifts.
- No weekend or on-call requirements.
- Must comply with COVID-19 and flu vaccine policies.
- Free choice of scrub colors.
- Mileage reimbursed according to IRS standards.
- Maintain excellent communication and coordination with the healthcare team.
- Ability to float between all facilities within a given state is expected.
Please note : This position does not permit the accommodation of block schedules or local rate differences within a 50-75 mile radius. If a communication device is required for assignment duties, and not provided, submission of monthly phone usage for work-related purposes is expected.
