Job Summary: The Registered Nurse - Case Manager provides comprehensive case management and care coordination for a diverse panel of patients with varying needs, spanning across different ages, medical conditions, and populations. The Case Manager coaches patients to improve their health behaviors and achieve their health-related goals, while working as part of a collaborative healthcare team to ensure the best possible care outcomes.
Key Responsibilities: - Care Coordination: Collaborate with healthcare providers, nurses, social workers, patients, families, and caregivers to ensure comprehensive, holistic care for patients.
- Patient Coaching: Educate and coach patients on health management and behavioral change to help them attain personalized health goals.
- Performance Recognition: Acknowledge and reward team performance in a challenging healthcare environment to maintain motivation and morale.
- Leadership & Mentoring: Provide guidance and mentorship to colleagues, offering clear direction for success in their roles and helping those interested in advancing to other positions.
- Care Documentation: Maintain accurate and up-to-date patient records, ensuring compliance with healthcare regulations and best practices.

Required Qualifications: - Education: Graduate from an accredited school of nursing.
- Licensure: Active, unrestricted licensure as a Registered Nurse in the state of practice.
- Travel Requirements: Current state driver's license with the ability to travel to hospitals, skilled nursing facilities, patients' homes, and other care sites as needed.
- COVID-19 Vaccination: Full COVID-19 vaccination is an essential job function; compliance with state booster requirements is necessary.

Preferred Qualifications: - Certifications: Certification in Geriatric Nursing or Geriatric Case Management is preferred.
- Experience: 5+ years of nursing experience, preferably working with older adults or geriatric patients.
- Technical Skills: Proficient in computer use, internet navigation, and health information technology (e.g., Electronic Health Records).
- Interpersonal Skills: Strong communication, interpersonal, and organizational skills with a flexible, patient-focused approach to problem-solving.
- Teamwork: Proven ability to work effectively both independently and within an interdisciplinary team setting.
