Position Summary:
Responsible for comprehensive assessment, evaluation, planning, administering and directing patient care services applying evidence based practices (EBP) principles. Uses the American Physical Therapy Association’s, Guide to Physical Therapist Practice as the foundation for patient/client management. Always provides “patient-centered” excellent customer service and clinical care, which address patient/family needs, preferences, goals, and assists patients in reaching their maximal level of performance and function while learning to live within the limits of their capabilities. Document all aspects of care/services, compliant with department policy, professional association, payer source, state, and federal regulatory guidelines. Inpatient focus-experience preferred. Rehabilitation Services is both a beautiful, new 5000 sq. ft. outpatient adult ortho-neuro and aquatic center. Rehab is adjacent to a 100-bed hospital that is a level 3 trauma center with CMS 4-star; CVMC is also certified by the Joint Commission in Bone and Joint and Hip Fracture pathways. The Rehab team consists of PTs, PTAs, OTs, COTA and SLPs. PTs are doctorate trained with a FAAOMPT, vestibular certified, OCS, MLD, Loud/Big certifications and other specialty training allowing for mentorship, whether you are a new grad or an experienced therapist looking to specialize.

Minimum Qualifications:
- Bachelor's Degree is required.
- Master's Degree or Doctorate preferred.
- Must have an Active Physical Therapist License.
- Must obtain CPR certification within 90 days of hire.
