LifeStream is a behavioral health and social services organization that provides high-quality treatment, education, care management, rehabilitation, child welfare, primary care and homeless services to children, adolescents, and adults. Located in Central Florida, LifeStream primarily serves the residents of Lake, Citrus and Sumter Counties, with additional programs located in Orange, Marion, Hernando counties.
LPN Residential Manager
JOB SUMMARY: The Nurse Residential Manager establishes, administers, and directs the Road to Home program site. This is a self-contained residential program that includes a clinical, nursing care and direct support personnel which assumes responsibility for directly providing needed treatment, and support services to individuals classified as having Severe and Persistent Mental Illness (SPMI) with co-occurring SA disorders. The Nurse Residential Manager supervises the RTH Behavior Techs, Clinical team and nurses within the Road to Home Program (RTH).

  • Be knowledgeable regarding the Baker Act, Level IA/IB Residential treatment facilities
  • Be knowledgeable of mental health diagnoses and the general attributions of the SMI population
  • Maintain open communication and respect with internal and external stakeholders (e.g. NEFSH discharge planners)
  • Adhere to residential protocol as addressed in Chapter 65E=4.016
  • Assist in networking resources and referral services for consumers
  • Maintain center records in accordance with stated policies and procedures.
  • Maintain consistency and accountability in consumer/contract reporting requirements.
  • Maintain current with all staff development requirements for this position; Perform staff evaluations & monthly meetings with staff
  • Review referrals in conjunction with RTH Program Director for appropriate admission into the Road to Home Program (RTH)
  • Convenes/attends treatment team as required for ongoing assessing and coordination of services for clients, to include discharge planning and follow up services
  • A Adhere to the program budget, Understand the fiscal and reporting requirements of the program.
  • Develop resources and billing opportunities for program sustainability outside of grant funding.
  • Ensure data collection for agency and contracted program requirements. Cooperate in the data reporting activities at the program, agency, county, state and federal levels.
  • Perform on-call back-up duty for nursing staff in accordance with regulatory requirements.
  • Perform on-call supervisory duty for scheduling needs, staff supervisory and consultation and emergency purposes.
  • Direct day-to-day operations of the RTH program including (but not limited to) scheduling staff work hours to assure minimum regulatory coverage; Perform payroll duties by due dates; Ensure supplies are ordered and available
  • Active participate in recruitment and retention efforts of staff including interviewing and ensuring successful staff onboarding/training
  • Monitor and evaluate the status of persons served, ensure appropriate treatment planning and coordination of ongoing treatment activities, assist in the development of discharge plans and follow up care of all persons serviced.
  • Ensure all documentation performed by staff via EHR is completed in a timely manner
  • Ensure required outcomes measures and FASAMS requirements are completed in a timely manner for each person served.
  • Assist in the completion of requirements monthly monitoring reports and participate in monthly monitoring calls with appropriate managing entity.
  • Participate consistently in all in-services training programs (e.g. De-escalation Training)

  • Communicate and work collaboratively with medical providers for coordination of care

Must have a clear understanding of the characteristics and problems of youth with and co-occurring disorders and be knowledgeable about the child welfare system. Demonstrates excellent verbal and written communication skills. Works collaboratively with internal and external partners. Displays excellent telephone and customer service skills.
EDUCATION & EXPERIENCE :
  • Education :
    • Bachelor's degree or higher in nursing social work, psychiatric rehabilitation, counseling or psychology (Preferred)
    • Associate degree in Nursing (required)
    • Licensed Practical Nurse (LPN) required
  • Experience : Minimum of 6 years' experience with the co-occurring population and minimum of two-year supervisory experience required.
  • Driving Requirements:
    • Must be Over the age of 21 required
    • Minimum of 5-years driving experience required
    • Possess a Valid FL Driver's license (within 30 days of hire if currently do not possess)
    • Possess a continuously valid US Driver's license for the past three years, from date of issue required
    • Safe Driving record required
