Our Care Coordinators provide comprehensive care coordination including managing patient care and coordinating patient treatment plans.Here are some of the duties you will be responsible for: Handle all administrative tasks and duties for the physician/providerServe as the liaison or coordinator for the patients medical careStreamline all patient-physician communications to ensure patient satisfactionProvide medical literature and clinical preparation instructions to patientsAssist patients with questions and/or concerns regarding proceduresSchedule all procedures to be performed by the physicianReview the physicians schedule for maximum scheduling efficiencySchedule all diagnostic tests, procedures and follow-up appointmentsObtains all authorizations for procedures and testsCall patient to confirm procedures a week in advanceSchedule follow-up appointments including recallsCheck-out patients at the end of their visit and provide next step instructionsRequest medical records from doctors and hospitalsReturns patient calls promptly and professionallyCall-in new prescriptions and refills and obtain authorization if necessaryObtain lab results including stat requestsComplete tasks from Electronic Medical RecordReviews open orders every three days and works accordinglyContact patients with test resultsSends history and physical forms to outpatient facilityOther duties as assignedMinimum Requirements:High school diploma or GED equivalentCertified Medical Assistant (AAMA) preferred2+ years experience as medical assistant requiredMedical terminology knowledgeBilingual (English/Spanish) requiredAbilities Required:Multi-taskerEffective communicatorPoliteMedical terminologyPositive AttitudeTeam playerWorks well under pressureOrganizedSelf-directedMust be willing to cross trainGastro Health is the largest gastroenterology multi-specialty group in the country.
