Monitors patient and family satisfaction. Responds to questions and complaints from patients, family members, and payors regarding care. Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources. Provides discharge education and resource referrals to patients. Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.Qualifications - External Minimum EducationGraduate from an approved School of Nursing.Minimum Work ExperienceFour (4) years of clinical experience with Associate's Degree orTwo (2) years of clinical experience with BSN orTwo (2) years of hospital case management experience.Certifications AHA Basic Life Support (BLS) within 30 days of hire.Case Management Certification recommended or obtained after working under the supervision of a Certified Case Manager for a minimum of two (2) years.
