Job Description:
- Support the treatment of illness and/or injury through the use of physical exercises and therapies.
- Assist in the rehabilitation and restoration of body functions to promote maximum client wellness and independence.
- Provide therapy programs for patients of all ages with developmental deficits, physical injury, or illness.
- Coordinate physical therapy services and provide assigned clients with direct therapeutic care.
- Teach rehabilitation skills to clients and/or caregivers to promote maximum client functionality and safety.
- Perform preventive and rehabilitative physical therapy procedures individualized to the client and according to age-specific needs.
- Develop and revise a plan of care based on assessment, client history, diagnosis, physician orders, expected outcomes, and client goals.
- Apply treatments involving physical agents such as heat, water, and electricity using equipment such as whirlpool baths, electrical muscle stimulation, and ultrasound.
- Follow utilization guidelines and protocols.
- Formulate and implement appropriate teaching plans and communicate significant clinical findings and changes to the physician and other involved personnel, including clients.
- Document all skilled and medically necessary care thoroughly, promptly, and accurately.
- Ensure compliance with Medicare, State, Federal, JCAHO, insurance, payer requirements, and local policy.
- Maintain client and organizational confidentiality at all times.
- Participate in the Quality Management program and complete all required in-service programs to ensure standards are met and skills remain current.
- Conduct supervisory visits for Physical Therapy Assistants and Home Health Aides, prepare and update their written instructions as applicable.
- Participate in staff meetings, case conferences, business development meetings, and customer service programs as required.
- Deliver a high level of customer service to all internal and external customers.
- Demonstrate leadership competencies through communication and relationship building, developing self and others, and teamwork.
