We are looking for an exceptional Home Care Physical Therapist to join our team and make a real difference in the lives of those recovering from illness and injury. Work primarily weekdays with a weekend rotation monthly. Enjoy the autonomy of managing your daily work schedule
About the Role:
As a Home Care Physical Therapist, you will:
  • Provide both direct and indirect patient care to facilitate the rehabilitation and restoration of normal body functions.
  • Implement personalized treatment programs based on each patient's plan of care.
  • Administer therapeutic modalities and train patients in therapeutic exercises, transfers, and gait.
  • Supervise and guide PT Assistants and PT Aides to ensure the highest quality of care.
Responsibilities
  • Physical Therapy Assessment and Skilled Treatments.
  • Client Care Coordination.
  • Teaching, Supervision and Counseling Clients and Caregivers.
  • Documentation of Interdisciplinary Communication Supervision of Ancillary staff.
  • Problem Solving and Critical Thinking Skills. Documentation.
  • Ability to effectively and safely transfer patients with significant physical and/or mental impairment.
  • Time Management Development of Plan of Care

Qualifications
Ability to assist in patient lifts, transfers, and guarded functional activity training.
Minimum required: Bachelor's
Preferred/desired: Master's degree or doctorate
Competencies Skills
Essential:
* Clear Communication Skills Both Written And Verbal
* Able To Keep Confidential Information Regarding Patients, Team Members
* Able To Withstand Crisis Situations
* Has Skills To Provides Customer Service To Patients, Team Members And Visitors
* Proof Of Current/Valid Car Insurance
* Knowledge And Experience With Electronic Health Records
* Experience With Excel, Power Point, Word, Visio, Etc.
* Valid DL With No More Than 3 Moving Violations
Credentials
Essential:
* PT - Physical Therapist
* BLS - Basic Life Saving certification
Education

* Bachelor of Science (BS)
