Responsibilities:
Summary
This position is responsible for facilitating the placements of program participants upon release from designated CDCR institution locations to CBP programs as identified by the receiving STOP Placement Office including LSUDD, LSUDT, FOTEP, transitional/sober living, and outpatient facilities/programs.
Primary Duties and Responsibilities
- Collaborates with Parole Officers to determine placement.
- Collaborates with Transportation Coordinator to ensure transportation to CBP program placement.
- Assists the Community Service Representative in collaboration with participants Parole Officer to determine placement.
- Receives, tracks and monitors referrals from Parole Officers.
- Conducts assessment to appropriately place participants based on individual needs.
- Assists the Community Service Representative in collaboration with community-based providers (CBP) to determine reception into program.
- Responds to Agents concerns and communicate effectively with CDCR when non-placements occur.
- Collaborates with CBP to determine reception into program.
- Ensures participants have necessary medication and/or supplies for basic needs prior to entering facility.
- Completes required reports as required by CDCR and Program Manager
- Performs other duties as assigned.
Qualifications:
Minimum Requirements
- High school diploma or equivalent.
- Experience with case management and criminal justice populations is preferred.
- Ability to understand a variety of models including cognitive behavioral therapy, theories of addiction and dynamics related to substance use and other criminogenic risk factors.
- Knowledge of the California Treatment Initiative, in-custody and reentry treatment continuum of care, and the critical nature of portal-to-portal transportation in the retention process of participants.
- Extensive knowledge of the transitional process, both documentation and operational, between in-custody and community-based modalities/programs.
- Excellent knowledge of English and spelling.
- Possess a level of maturity to deal with sensitive issues inherent in the delivery of reentry substance abuse treatment services to the criminal justice population.
- Ability to know and comply with program documentation requirements, applicable ethical and professional standards of conduct
- Excellent communication skills.
- Excellent organizational and time management skills.
- Ability to read and interpret documents such as government regulations, legal documents, operating instructions, and procedure manuals.
- Ability to write routine reports and correspondence
- Ability to establish rapport with participants including management of crisis situation and determination of need for additional professional assistance.
- Ability to provide effective oversight of participants.
- Ability to facilitate the participant's engagement in the treatment and recovery process.
- Possession of the case management skills necessary to implement the CBP program placement, consult as a member of a multi-disciplinary team, and assess ongoing participant needs.
- Ability to work with computers and the necessary software typically used by the department.
