Qualifications and Requirements:

• At least one year of home care experience preferred
• Manage and oversee all aspects of care planning process including acting as liaison between caregivers and clients
• Assist in facilitation of new client on-boarding process
• Ensure compliance and quality assurance standards are met
• Positive attitude, great communication skills, and ability to work independently are required
