POSITION SUMMARY
Responsible to the Director of Professional Services/Clinical Manager to provide medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, company policies and professional Standards of Practice.
JOB RESPONSIBILITIES
Participates in the development of Patient physical therapy plans of care. Makes visits and communicates with the physician before start of care as needed. Documents the care and submits paperwork in an accurate and timely manner to update and maintain the Patient medical records. Provides education to Patients and their families. Participates in regulatory scheduled interdisciplinary team meetings to coordinate the care of the Patient and agency and family, exchange information and problem solve, and receive staff support and education. Applies the policies and procedures of the agency and the rules and regulations of Federal and State Regulatory agencies and The Joint Commission in providing care. Completes paperwork in an accurate, legible manner and adheres to agency policy for timely submission.
REQUIRED EXPERIENCE, EDUCATION, SKILLS & ABILITIES
- Currently licensed as a Physical Therapist (PT) in this state and in good standing with the Board in which he/she will practice.
- Knowledge of principles, concepts and methodology of physical therapy.
- Ability to work well with an interdisciplinary team.
- Ability to develop and maintain rapport with the Patient and family.
- Excellent written and verbal communication skills.
- Demonstrate problem solving and decision-making abilities.
- At least one year or more work experience in physical therapy in a multidisciplinary setting is preferred.
- KINNSER / Well Sky experience preferred
REQUIRED CREDENTIALS
- Current CPR
- Level 2 Background screening
- Recent negative TB / CXR
