Job Title: Physical Therapist - Rehabilitation Therapy
Job Specialty: Physical Therapy
Job Duration: 13 weeks
Shift: Tuesday - Saturday. First week will be Monday-Friday for orientation
Guaranteed Hours: 36 hours
Experience: Minimum 1 year
License: Vermont PT licensure
Certifications: Home Health experience and OASIS experience required
Must-Have: McKesson/Netsmart EMR experience
Job Description:
- Provide one-on-one therapy care for patients in their home setting
- Assess individual conditions and develop personalized care plans to enhance mobility and independence
- Collaborate with interdisciplinary teams to meet patient needs
- Perform standard therapy measurements such as ROM, strength, and balance tests
- Use therapeutic modalities to optimize patient care
