As an RN Case Manager, you will be a vital member of our healthcare team. You will be responsible for coordinating patient care, ensuring efficient and effective treatment plans, and promoting best practices in infection control. While infection control experience is preferred, it is not required. We provide extensive training and support to help you succeed in this role.
Key Responsibilities:

• Develop and manage individualized care plans for patients, ensuring they receive the highest standard of care.
• Monitor and evaluate patient progress, making necessary adjustments to care plans to achieve optimal outcomes
• Collaborate with healthcare providers, social workers, and other team members to coordinate comprehensive patient care.
• Educate patients, families, and staff on healthcare practices and infection prevention protocols.
• Participate in quality improvement initiatives and contribute to the development of patient care policies.
• Conduct regular assessments and provide support to ensure compliance with infection control standards.

Qualifications:

• Current RN license in MINNESOTA ,
• Infection control experience preferred but not required.
• Strong knowledge of healthcare standards and best practices.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work effectively both independently and as part of a team.
• Compassionate and patient-centered approach to care.
