Key Responsibilities

• Administer and monitor medications with accuracy and safety in skilled nursing
• Perform vital sign checks and health assessments to ensure effective health
• Develop and implement personalized care plans with the healthcare
• Provide patient-centered care, assisting with daily activities and fostering
• Educate residents and families on health management, wellness, and
• Document care and maintain accurate patient
• Apply expertise in medication administration and personalized care
• Reassess and adapt personalized care plans to evolving resident
• Conduct routine vital sign checks and update health monitoring

Required Qualifications

• Active RN license in
• Minimum 1 year of experience in long-term care, skilled nursing, or
• Expertise in medication administration, vital signs monitoring, and health
• Strong communication and teamwork
• Ability to lift 50 lbs and work day shifts (10:30pm-6:45am).
• Experience as a Registered Nurse in skilled nursing
