    Certification Details
  • Vaccination series completion or declination documentation required
  • Booster vaccine may be required if titer is negative
  • Annual Fit Test required
  • Annual update of physical exam or health statement
  • Proof of residency matching with state of compact license for RNs is needed if assignment state is different
  • Evidence of immunity required for varicella
  • Documentation of Tdap vaccination valid for 10 years

  • 12 weeks - EOW; EOH; 2 years PCU / Med-Surg Telemetry Acute Care experience required; may need to float onsite within skill set or to sites within 40 miles if needed
