We are mindful of the past yet focused on the future.nnTheLPN oversees Medication Administration, monitors client health, and collaborates with program staff around client treatment.The hourly rate for this role is $24.00/hr - $39.00/hr.nnRESPONSIBILITIES :nnFacilitate the monitoring of the client's self-administration of medication.nnAppropriately document all medication taken by residents.nnMonitor clients for any adverse reactions to medications and take appropriate actions, and report as necessary.nnAccept and document deliveries of medication.nnPerform daily medication reconciliations to verify counts of all clients' medication.nnComply with all medication policies and procedures.nnSecure, monitor, and distribute controlled medications, in compliance with all agency, city, state, and DEA regulations.nnCoordinate with other nurses and staff to coordinate patient care and medication.nnDocument and share all non-compliance with medication regimens.nnAppropriately discard medication as required, do so and document in accordance with all policies and regulations.nnProtect client confidentiality except in cases to coordinate care with staffnnConduct groups with residents the promote medication education and medication compliance.nnCoordinate processes to improve/evaluate medication administration. Makes recommendations.nnAssist with referring clients to appropriate healthcare providers/specialists.nnCoordinates with the client's primary care physicians to inform decisions regarding the management and treatment of problems identified and provides care as prescribed by a physician.nnAssist with the management of client prescriptions with medical providers and various pharmacies, with a focus on Evers Pharmacy.nnProvide oversight and monitoring to promote organized and efficient medication tracking, refills, and disposal of medication, as needed.nnOversee compliance with rules for documentation of medication administration.nnIdentify when a person's response to a medication or change in condition requires medical review and possible intervention. Notify medical providers of this observation.nnConsult with Medical Director/Provider if a self-directed individual appears to no longer be capable of taking or understanding their medication.nnCollaborate and coordinate with 819 provider staff and provide reports as requested.nnParticipate in client clinical review meetings and coordination of care meetings as requested.nnMonitor and document clients' vital signs and additional vitals as needed to ensure client safety i.e., sugar levels, blood oxygen levels, etc.nnPromptly investigate medication errors based on severity index/potential severity to improve staff performance and patient care. Work with Medical Director to identify and when required, report errors to the proper authorities.nnParticipate in meetings with staff and vendors that work to improve policies and practices.nnServe as a resource person by consulting with agency staff.nnPerforms other tasks as assigned by Supervisor/s.nnEDUCATION AND REQUIREMENTS:nnRegistered Nurse from an accredited Nursing school with appropriate educational degrees.nnA minimum of one (1) year of experience as an LPN.nnMust possess an active and unrestricted license as a Licensed Practical Nurse (LPN) in New York State.nnMust possess current Basic Life Support (BLS) certification.nnMust possess current NYS-approved Infection Control certification (4.0 hours, valid for 4 years).nnKnowledge of substance/alcohol dependence/use population.nnMust have specific training in chemical use, abuse, and dependence specific to the services provided.nnObtain within six months of hire:nnSupporting Recovery with Medications Addiction Treatment (MAT)nnCASAC Canon of Ethics (6 hours)nnConfidentiality related to 42CFR (3 hours)nnMandated Reporter (2 hours)nnAll procedures and functions shall be conducted in accordance with NYS OASAS Local Services Bulletin No. 2012-04: Medication Administration Policies for Administration of Medications in OASAS Intensive Residential Programs issued December 14, 2012, and newer as updated by OASAS.nnAbility to utilize and operate electronic record systemsnnAcacia Network is an equal opportunity employernnJob Requirements
