We are currently looking for an anticoagulation nurse to work in our outpatient Anti-Coagulation ClinicJob Summary:Responsible for utilizing the nursing process to provide comprehensive care appropriate to the age of the patients served by the Anti-Coagulation Clinic, as directed by the Clinic`s Medical Director. Nursing care is provided in accordance with the stated philosophy and standards established by the Nursing Service Department, regulatory agencies and professional organizations. The Registered Nurse is accountable for the safety of nursing care they deliver or supervise. Responsible to the patient and the family or significant others for the quality of direct and indirect nursing care delivered. Line responsibility to the Director of the Pharmacy and/or designee.Job Functions:1. Assures the safety of the patient population served by demonstrating age-appropriate skills in assessing, planning, implementing, and evaluating anti-coagulation management. 2. Obtains INR result by performing finger stick INR test, or obtaining result from appropriate laboratory. 3. Utilizes clinic guidelines to determine appropriate anticoagulant dose. 4. Performs focused history and physical exam to determine response to therapy, presence of adverse outcomes or other medical problems 5. Communicates INR, dose instructions and next appointment to patients. 6. Documents details of the encounter in patient record. 7. Provide comprehensive patient education with reinforcement as needed. 8. Utilizes clinic guidelines to develop plan of care for perioperative patients requiring interruption of anticoagulation. 9. Administers anticoagulation medication and/or teach patients to self-administer as appropriate. 10. Provides care for inpatients referred for anticoagulation management. 11. Provides care for patients referred for outpatient DVT protocol. 12. Provides comprehensive education and appropriate follow up for patients referred for INR self-testing. 13. Assisting in the training and orientation of new staff. 14. Performs phlebotomy as required. 15. Maintains and supports patient's rights to care, privacy and respect. 16. Other duties as assigned from time to time.Minimum Qualifications:• Graduate of professional accredited nursing school required.Experience: • Minimum two years' critical care or cardiology experience required.• Previous anti-coagulation specific experience strongly preferred Licensure and/or Certification:Currently licensed to practice professional nursing in Massachusetts required. BLS healthcare provider requiredShift:Days8:00am-4:40pm or 8:30am-5:00pmNo Call requirement except for Q3 to Q4 weekend (4 hours only on Sunday)No HolidaysRate: $95
