Position Highlights
- Department: Pharmacy - Anti-Coagulation Clinic and Inpatient Pharmacy
- Hours: Full-Time/40 hours (24 hours in the Anti-Coagulation Clinic and 16 hours Inpatient Pharmacy)
- Shift: This position will primarily work day shift hours, with some evening shift hours required. Night shift coverage is minimal and based on departmental needs. Working every third weekend is required, as well as on-call responsibilities
Position Summary:
The Ambulatory Care Pharmacist in conjunction with the Director of Pharmacy and pharmacy leadership in accordance with established policies and procedures, will provide medication therapy management to provide the safest and highest quality care for our patients. This position will also be cross-trained to cover the inpatient pharmacy as a clinical staff pharmacist.
Approximately 24 hours weekly will be dedicated to outpatient anticoagulation clinic, and 16 hours weekly dedicated to inpatient pharmacy.
- Participate in anticoagulation management for patients under collaborative practice agreement
- Think critically and problem solve in clinical settings to provide best patient outcomes
- Follow clinic policy and protocol to provide best patient care
- Communicate with other healthcare professionals to maximize treatment efficacy and safety
- Opportunities to precept APPE students and pharmacy resident
- Educate patients and caregivers on proper medication therapy management
- Clinically review medication orders to ensure patients are receiving medication therapy according to the most up to date clinical standards
- Provide therapeutic drug monitoring and follow up for patients
- Compound, dispense, and verify medications, including sterile compounding
- Support continuous improvement and growth of pharmacy practice
- Solves practical and strategic problems
Minimum Qualifications:
- Registered pharmacist in good standing with the state of Connecticut
- B.S. or PharmD. degree
- Good patient, provider and staff communication skills
- Strong commitment to working with patients and staff from a wide range of ethnic, economic, cultural and social backgrounds
- Strong interpersonal skills including the ability to handle internal and external issues in a calm, positive, and professional manner
- Ability to maintain a professional demeanor and confidentiality
Preferred Qualifications:
- Board Certification Ambulatory Care Pharmacist (BCACP) or other specialty
