Compass Healthcare Consulting & Placement is conducting a search for a Hospice Nurse, RN Registered Nurse making visits to Hospice patients. Competitive Salary & Excellent Benefits. Position requires travel to one of the following areas: Manhattan, Bronx or Westchester County, NY. Qualified candidates will have prior experience in medical-surgical, community health, and/or hospice, palliative care, oncology nursing, preferred. Hospice Nurse, RN Assists in the planning, implementation, documentation and coordination of patient care services to Hospice patients and their families. In this field based position, you will visit Hospice and Palliative Care patients to perform both physical and psychosocial assessments and document visit occurrence. As the Hospice Field RN, you will develop and implement nursing plans of care and make recommendations for care based on patients' needs. As needed, you will contact the patient's physician to request written orders to facilitate plan of care changes. In addition, you will also supervise, train and evaluate home health aides and homemakers working with the hospice patients. Responsibilities:Provides direct nursing care to patients.In collaboration with the Interdisciplinary Team, coordinates comprehensive nursing services for patients and their familiesEvaluating and initiating patient care pursuant to the physician’s plan of care.Re-evaluating the effectiveness of the patient’s plan of care and makes revisions based on the patient/family changing needs.Arranging for other agency services as needed to meet the patient/family care needs.Collaborating with the Interdisciplinary Team on all aspects of patient/family care.Documenting all pertinent information in the clinical record including evidence of coordination of care for an assigned group of patients.Consulting with the patient/family to help them participate in the mutually established plan of care.Informs the physician and other Hospice personnel of any changes in the patient's condition.Obtains medical orders.Prepares written instruction for and supervises the Home Health Aide.As scheduled, is responsible for being on-call in accordance with Hospice policies and procedures.Participates in Interdisciplinary Team meetings, Hospice in service, volunteer training and community education programs.When needed may be required to coordinate intake for the Hospice.Assists in the orientation of new staff.Assists in the explanation of Hospice eligibility criteria and reimbursement sources.Participates in quality assessment and improvement activities.Performs related duties as assigned.Works within legal, ethical practice standards to his/her position as well as Company Code of Conduct.Complies with appropriate policies and procedures and brings potential compliance matters to the supervisor or compliance officer. Qualifications:Current NYS Registered Nurse license.Baccalaureate Degree in Nursing from an accredited school is preferred.Two (2) years experience in nursing which includes any combination of medical-surgical, oncological, palliative and community home health/hospice.At least one (1) year of community hospice or home health experience is preferred.Hospice experience preferred.NYS driver’s license preferred.
