A Federally Qualified Health Center (FQHC) in New York is actively seeking a dedicated Nurse Practitioner (NP) to join their team. This role involves providing comprehensive primary care, managing chronic conditions, and collaborating with healthcare providers to improve health outcomes for a diverse patient population.Responsibilities:Provide comprehensive primary care services, including diagnosis, treatment, and follow-up care for patients of all ages.Develop individualized care plans and manage chronic conditions such as diabetes, hypertension, and asthma.Educate patients and their families on health conditions, preventive measures, medications, and healthy lifestyle choices.Qualifications:Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program.Board certification as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or other relevant NP certification.Experience in a community health, primary care, or FQHC setting.
