Talent Healthcare Staffing Inc is seeking a travel nurse RN Case Management for a travel nursing job in Seattle, Washington.Job Description & RequirementsSpecialty: Case ManagementDiscipline: RNStart Date: 01/27/2025Duration: 12 weeks40 hours per weekShift: 8 hours, daysEmployment Type: Travel Primary Responsibilities/Accountabilities: \t Conducting intake assessments with patients and families. \t Ensuring that families feel sufficiently equipped and supported. \t Developing individualized treatment plans that account for physical, financial, and other constraints. \t Adjusting treatment protocols, as needed. \t Suggesting and coordinating appropriate follow-up services to promote wellness. \t Ensuring that patients have transport to and from their homes and all pertinent healthcare facilities. \t Meeting with members of healthcare teams to discuss patients' progress, and to deliberate on appropriate care. \t Checking to see whether patients have sufficient insurance coverage or finances to pay for their treatment. \t Devising innovative solutions or suggesting appropriate referrals for patients who cannot afford to access treatment. Qualifications: \t Bachelor's degree in nursing. \t Advanced degree in nursing is strongly advantageous. \t Certificate in community and/or hospital case management. \t Prior experience as an RN case manager within a similar healthcare facility. \t Driver's license and own vehicle. \t Familiarity with patient databases. \t Strong understanding of ethics surrounding the storage of patient information. \t Exceptional leadership, supervision, and motivational skills. \t Committed to promoting patients' well-being. \t Ability to work evenings, weekends, and holidays.
