The largest not-for-profit Program of All-Inclusive Care for the Elderly (PACE) in the nation is looking for a Nurse Practitioner for their brand new mobile program. Van starts in Bronx and will travel to Jamaica, Queens each day for majority of work - could also go to different events company hosting, be flexible to travelWill perform annual wellness assessments on community participants and reflects an understanding of the complexity and multiple comorbidities of the frail and/or elderly - participants with higher levels of risk, these assessments will be performed more frequently to appropriately address his/her level of needMonday to Friday 8-5pm schedule (40 hour work week, generally will travel from 8-9 and 4-5 so only seeing patients from approx 9:30-4pm each day)Responsible for revieing patients' medical history every 6 months and assesses those that were hospitalized or sent to EDWill be responsible for identifying active diagnoses and chronic problems or conditions to be used in Care Management and active medical management of treatment and designed interventionsResponsible for supervising the care plan and delivery by each provider - needs to be tailored to the needs of the individual and must be prepared to intervene as neededJob RequirementsNY State license, DEA, CSR (active or applied preferred) but not requiredMasters as an Adult or Family Nurse Practitioner Board Certification RequiredPrevious experience in geriatrics, home care or care management requiredDrivers license requiredDisclosure: The hourly rates and/or salaries listed may or may not reflect total compensation packages including bonus and fringe benefits, etc., nor are the advertisement(s) posted a guarantee of a certain compensation package for a position or bona fide offer of employment.
