Responsibilities may include the following but are not limited to:
n
n- Traveling to patient's home and providing "Best Practice" nursing care
n- Checking vital signs
n- Documenting in a complete and confidential manner
n- Evaluate the patient’s wellbeing, reporting any needs to the Director of Clinical Nursing or emergency services
n- Providing personal care and meals
n- More advanced nursing such as chest tubes, IVs, colostomy, urinary catheter management are also available
n- Giving emotional support to client and families in times of crisis
n

Requirements:
n
n- Valid LPN/RN license
n- Annual physical within one year
n- PPD or Quantiferon within one year or chest x-ray within five years
n- MMR labs
n- COVID vaccine card with booster
n
