Responsibilities
n
n- Facilitation of rehabilitation of patients with emotional, physical or mental disabilities
n- Implementation of occupational therapy care plans
n- Coordination of care with entire care team
n- Educating patients and family members
n- Preparing timely, complete and accurate clinical notes
n- Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members
n

Qualifications
n
n- Believes in a team approach to problem solving and care Freely
n- Physical Therapist in the State of Texas
n- Skilled Nursing, Healthcare (Long Term Care) or Veterans Home experience highly preferred
n- Knowledge of Home Health regulations and general healthcare compliance
n- Experience with EMR
n- Truly cares for people and will go above and beyond to provide outstanding patient care
