Performs assessments in collaboration with the physician and as needed.nnDevelop and implement appropriate plans of care to PO participants.nnEvaluates participant physical complaints and provides appropriate treatment.nnProvide participants and caregiver teaching and education.nnFunctions as a member of the Interdisciplinary Team (IDT). Regularly informs the IDT of the medical condition of each participant, remaining alert to pertinent input from other team members, participant’s caregivers, as well as documenting changes in a participant’s medical record consistent with documentation policies.nnWhat you will need:nnMaster's degree in NursingnnFive (5) to seven (7) years of experience required preferably in long-term care.nnMust possess a current, unencumbered, active license to practice in the state of PA.nnCDS Certification and DEA Registration required.nnBoard Certification by ANCC or AANP.nnCertification as a Nurse Practitioner with prescriptive authority required.nnCurrent BLS required.nnA minimum of one (1) year working with the frail or elderly population strongly preferrednnMust be action-oriented, be customer focused, have high decision quality, flexibility to adapt to ongoing change and have organizational agility.nnMust have a valid driver’s license, proof of insurance and have means of transportationnnProficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring systemnnPosition requires periodic travel within the Centers geographic region.nnPosition requires frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting.
