Summary:
The Registered Nurse, Travel (Travel Nurse) works under the supervision of the Director of Continuity of Care or designee and the attending physician to coordinate all aspects of the patient's care in accordance with current professional standards and practice to maximize the comfort and health of patients and families. The Travel Nurse assesses, plans, implements, and evaluates the care of hospice patients in the home care, skilled nursing facility, and inpatient settings. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician's orders and plan of care and Care Dimensions policies and procedures.
The Travel Nurse is responsible for patient assessments and admissions to Care Dimensions, ensuring continuity of care between the admitting process and transition to ongoing care by the hospice interdisciplinary team (IDT). Additionally, the Travel Nurse is responsible for educating health care professionals, patients, and families in all settings regarding the benefits/appropriateness of hospice care.
The Travel Nurse maintains good communication with Care Dimensions clinical staff in order to enhance hospice services and facilitates communication between Care Dimensions and referral sources.
What You'll Bring:

  • Registered Nurse, currently licensed in Massachusetts in good standing.
  • Associate's degree in nursing required; Bachelor's degree in nursing, or equivalent preferred.
  • Minimum three years of varied work experience as a professional nurse, and preferably one of the three years in an acute setting.
  • Experience in the use of electronic medical record preferred.
  • Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
  • Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
  • Demonstrated understanding of hospice philosophy and principles.
  • Demonstrated understanding of Medicare/Medicaid benefit as it relates to all facilities settings.
  • Demonstrated ability to be self-directed, flexible, and cooperative in fulfilling role obligation and ability to work effectively within an interdisciplinary team.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and in good working order.

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post. Pay Range
$40-$52 USD
