Responsibilities
n
Clinical Excellence & Patient Care
n
Possesses the knowledge and clinical skills necessary to provide care for people with physical disability and chronic illness.
n
Participates in patient/family educational activities and uses appropriate resources to implement an individualized teaching and discharge plan with patients and their families.
n
Performs hands-on nursing care by utilizing the nursing process to achieve quality outcomes for patients.
n
Reinforces the care provided by other team members.
n
Provides direction and supervision of unlicensed assistive personnel (e.g., Patient Care Technician), demonstrates clinical judgment appropriate for the states LPN scope of practice, uses problem solving techniques and time-management principles, and delegates appropriately.
n
Coordinates nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team to facilitate achievement of overall goals.
n
Demonstrates effective oral and written communication skills to develop a rapport with patients, their families, and health team members and to ensure the fulfilment of requirements for legal documentation and reimbursement.
n
Maintains a strong commitment to the multidisciplinary health care team.
n
Contributes to the assessment of the physical, psychological, sociocultural, and spiritual dimensions of patients and their families, as well as their educational and discharge needs.
n
Collaborates with other rehabilitation team members and contributes to the dynamic interdisciplinary plan of care.
n
Implements the interdisciplinary plan of care within the scope of LPN/LVN practice and provides nursing care and education directly or through ancillary personnel, as needed, to maintain and restore function and prevent complications and further loss.
n
Contributes to the evaluation of the nursing care and to the modification of the plan as needed to achieve measurable goals and objectives.
n
Contributes to the interdisciplinary team process at team conferences and other team meetings and offers input into team decision making within their scope of practice.
n
Intervenes with team members and other healthcare professionals to ensure that the optimal opportunity for recovery is made available to the patient.
n
Collaborates with the staff nurse and other team members to achieve cost-effective care by utilizing appropriate clinical measures to meet emergent physical, psychosocial, and spiritual situations.
n
Communicates effectively and interacts in a manner that facilitates the group process and team building.
n
Advocates and supports the rights of patients and families with care decisions.
n
Practice reflects knowledge of federal/state regulatory and accrediting agencies (e.g., Department of Inspection and Appeals (DIA))

Clinical Excellence & Patient Education
n
Reinforces information provided to patients about the disease processes underlying disabilities and teaches nursing techniques to help patients and their families develop the self-care skills necessary to move toward wellness on the illness-wellness continuum.
n
Provides assistance to patients and their families within his/her scope of practice for future self-management and decision-making responsibilities by fostering patients’ independence and goal achievement.
n
Reinforces the teaching done by the staff nurse and other specialists and other healthcare disciplines and provides resource materials for patients’ changing needs.
n
Demonstrates knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately.

Qualifications
n
Education:
n
Successful completion of a Licensed Practical Nurse program from an accredited college.
nn
Experience:
n
Experience in Post-Acute Care or an Acute Care environment.
nn
License(s)/Certification(s):
n
Current licensure in good standing to practice as a Licensed Practical Nurse within the state you are practicing.
n
Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three (3) months from date of hire.
