Job Description:
Growing Geriatric Internal Medicine practice is looking for a Nurse Practitioner to join our team in providing comprehensive primary care services to older adults in Colorado Springs.
AgeWell Medical Associates is a primary care practice that specializes in providing comprehensive medical care to seniors aged 60 and above. AgeWell Medical Associates consists of two physicians, four APP's and a psychologist providing on-site geriatric behavioral health support. The practice caters to seniors with varying health conditions, ranging from those who are well to those with multiple chronic conditions. Patient-centered care is the practice's north star, with the goal of helping patients achieve their health goals.
Our practice is a member of the independent practice association (IPA), PHPprime. PHPprime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.

We are seeking an experienced NP and one who has an interest in building his/her own ambulatory care practice seeing patients in our office Monday - Friday. Candidates should be able to work collaboratively with the physician(s) but with minimal direct physician oversight, have solid primary care skills with strong attention to detail, and enjoy working with adults age 60 and up.
