The NP is responsible for managing health problems and coordinating high-quality primary medical care in partnership and collaboration with Honor Community Health’s (HCH) integrated practice team, in support of our mission of delivering comprehensive integrated care to vulnerable populations of Oakland County in accordance with State and Federal rules and regulations and the nursing standards of care. The NP is responsible to the Associate Chief Medical Officer for clinical quality and outcomes, as well as productivity.
