Job Description
Minimum Qualifications/Experience:
- Bachelor’s degree in public health, nursing, social work, or a related field, with two years of relevant experience; OR LPN with 5+ years of healthcare experience.
- Additional graduate coursework may substitute for experience, and experience can substitute for education (up to four years).
- Candidates with experience caring for individuals with dementia are encouraged to apply.
Job Overview:
Promote dementia-friendly concepts, provide community memory screenings, educate the public on Alzheimer’s and dementia, encourage early detection, and foster community-clinical linkages.
Key Responsibilities:
- Build relationships with community partners and healthcare providers to support individuals with ADRD and caregivers.
- Offer educational materials, memory screenings, and resources.
- Track program success and barriers to care, ensure HIPAA compliance, and manage financial records.
