The goals of care coordination are to support the most appropriate and least restrictive setting, to improve the consumer's level of functioning and to assist the consumer an stakeholders in learning and retaining skills to help the consumer remain in the community. The Care Coordinator role at the CRU is delineated to review referrals to the facility and to assist the client in the discharge process.
