Potential Sign-On Bonus Eligible: $5,000 paid out over 1 year
This position is responsible to respond to the needs of adults with a Serious Mental Illness (SMI) or Children with Serious Emotional Disturbance (SED) using a patient-centered approach which removes silos of care and supports an integrated system. This position will provide whole-person care coordination across medical, behavioral, and social service community systems.

Responsibilities:
- Develop, implement, and monitor care coordination plans.
- Address treatment barriers for each low to moderate risk patient. Refer high risk patients to Nurse Care Manager.
- Educate members related to health information, referral to resources, etc. Communicate and coordinate services with other health and social service providers.
- Promote patients health and ensure that all personal health goals are included in person-centered care management plans.
- Conduct home visits, contact members by phone, attend meetings, staffing, care plan conferences, etc. related to enrolled members.
- Documentation of services using the Centers Electronic Health Record and other Provider Portals as required.
- Completion of health assessments and other requirements outlined by funders
- Maintain patient confidence and protect operations by keeping information confidential.
- Participation in meeting performance measures and continuous quality improvement activities, including data collection, tracking, and analysis.
- Adhere to all policies and procedures set forth by UnityPoint Health and other funding sources.
- Accomplish all tasks as appropriately assigned or requested.
- Assist in the recruitment and enrollment process of new members
- Collaborate with physicians, consumers, members, visitors, staff, and the health care community.
Qualifications:
Minimum Requirements:
- Bachelors Degree in a Human Service-related field
- Valid drivers license
Preferred:
- 1 year working with adults and/or children with mental illness
- Ability to form good working relationships with clients
- Knowledge of working with adults and children with mental illness.
- Knowledge of community resources
