Position Overview: As a Home Health Care Case Manager, you will be responsible for coordinating comprehensive care for patients in their homes, with a focus on acute care and wound care management. This role involves collaborating with healthcare professionals, families, and patients to develop and implement effective care plans. Your expertise will play a crucial role in promoting healing and enhancing the quality of life for our patients. Key Responsibilities:Assess patients’ needs and develop individualized care plans, particularly for acute health conditions and wound care management.Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient care.Monitor patient progress and adjust care plans as necessary.Provide education and support to patients and their families regarding wound care and overall health management.Maintain accurate and timely documentation of patient care activities and progress.Ensure compliance with all regulations and quality standards in home health care.Participate in interdisciplinary team meetings and case reviews. Qualifications:Wound care certified preferred but not required.Current RN or LPN license in Massachusetts.Previous experience in home health care or case management, with a strong focus on wound care.Excellent communication and interpersonal skills.Strong organizational skills and attention to detail.Ability to work independently and as part of a team.Proficiency in electronic medical records (EMR) systems.
