Position Summary The Department of Care Management utilizes the Team Leader model to enhance the operational and clinical effectiveness of the department and to ensure seamless management of the day to day activities of staff engaged in discharge planning, care coordination activities, social work and capacity management.
Responsibilities - Serves as clinical expert for staff within a designated area and acts as first level consultant for staff within designated team to address patient care issues, discharge barriers, etc.
- Advises staff appropriately and includes departmental leadership as necessary.
- Serves as point of contact and mentor for members of the designated team.
- Monitors census daily to ensure appropriate staffing and census distribution within the designated team.
- Schedules and conducts team meetings/huddles (weekly)
- Improves and sustains patient flow, length of stay and capacity management
- Coordinates with departmental leadership to optimize and facilitate onboarding of new team members, meets weekly with orientee and clinical coach to ensure successful transition through orientation process.
- Provides for educational support to residents, attending, and new medical students for respective designated assigned areas.
Minimum Requirements - Masters of Social Work degree from a CSWE accredited school of social work is required with minimum two years case management experience or a BSN in nursing from an accredited college or university required with minimum 2 years case management experience is required.
- If a Registered Nurse, current licensure as a Registered Nurse by the NC Board of Nursing is required.
- Certification in Case Management required within 2 years of hire is required.
- 1 year to 3 years required in Case Management/discharge planning, preferred acute care case management
