RN Observation Navigator Job Description

Position Summary
As an Observation Navigator , you will leverage your clinical expertise to assess patient needs and collaborate with multidisciplinary teams to ensure appropriate placement in Observation or Inpatient settings. You’ll focus on individualized care planning to enhance quality, optimize resource utilization, and promote patient satisfaction.
Key Responsibilities :
- Assess patient scenarios to determine appropriate Observation vs. Inpatient status.
- Collaborate with physicians, patients, families, and staff to create effective care plans.
- Monitor patient progress, reassess plans as needed, and ensure high-quality care delivery.
- Drive appropriate utilization of Observation services.
Schedule:
- Full-time
- Monday to Friday
- Day Shift
Education :
- Bachelor’s Degree preferred
- Diploma/Associate’s degree in Nursing required
Licensure/Certification :
- Active Florida Registered Nurse (RN) License
- Professional Case Management Certification (CCM) preferred
Experience :
- Minimum of 5 years’ experience in nursing, with Case Management and/or Utilization Review experience preferred
Skills :
- Strong communication skills to effectively engage with physicians, patients, families, and staff
