RIDGEVIEW, a not-for-profit community mental health center, is pleased to announce an opportunity to be a part of our Mobile Health Clinic to the right bachelor's level candidate. This innovative team-based program, which provides integrated primary care and behavioral health services to rural locations, is seeking a Case Manager/ Care Coordinator to serve as a key component of providing those services. This position will work weekdays during normal business hours with some flexibility.A highly specialized and customized vehicle (aka Mobile Health Unit) serves as the base of operations for the Case Manager as well as the other members of the team (i.e. Nurse Practitioner, Therapist, etc). The MHC multidisciplinary team travels to designated locations within Ridgeview's service area in order to provide increased access to basic health care, mental health and substance use treatment in historically underserved communities. The program utilizes a team-based model of care which includes mental health counseling, primary health care, case management/care coordination, supportive employment services and access to a peer recovery specialist.
