The YMCA of Pierce and Kitsap Counties is seeking a Care Coordinator to join our team. TheYMCAfocuses on empowering people in all forms, by improving health and well-being and inspiring action in and across our neighborhoods.This position supports the work of the Y, a leading non-profit, charitable organization committed to strengthening community through youth development, healthy living and social responsibility. As a Care Coordinator, you will be responsible for directly supporting the YMCA's care coordination process of connecting medically or self-referred individuals to the Y's suite of evidence-based chronic disease prevention and management programs and services. If hired, you will perform care coordination activities with individuals through engaging over the phone, via email, or other communication strategies as needed. You will also be expected to conduct in-person consultations and motivational conversations with individuals as needed. Key Responsibilities: Provide administrative, organizational, and logistical support to the Heath Initiatives Department.Assists Directors in quality assurance of chronic disease prevention programs.Maintains and interacts with a variety of electronic databases to record personal and/or protected health information. Enters and sends/receives data through the YMCA referral management system following all privacy and security protocols. Conducts care coordination conversations with referred individuals and other prospective community members utilizing motivational interviewing and active listening skills. Assists with marketing and retention activities to build robust engagement in YMCA's evidence-based services with high enrollment. Ensures accurate intake forms, data collection forms and appropriate program documentation are completed. Ensures that program/enrollment targets are met and consistent with program standards. Supports quality assurance and other administrative duties related to chronic disease programming across the YMCA of Pierce and Kitsap County Association.Embraces and actively promotes an inclusive and equitable work environment.Represents the YMCA and self in a professional manner, offering safe and engaging healthy lifestyle practices.Delivers program in accordance with evidence-based program fidelity and YMCA core values: honesty, respect, responsibility, and caring.Other duties as assigned.Are you a good fit?One or more years of work experience in activities connected to health promotion services, public health, case management, community outreach, or similar field. Working knowledge of preventative health programs and philosophy. One or more years of work experience in activities connected to health promotion Comfort interacting with individuals' health-related information and communicating with health care providers. Proficient to advanced computer skills and experience with Microsoft Office suite. Ability to drive and/or reliable transportation plans to work on-site at various YMCA centers and community organizations.Ability to respond to safety and emergency situations.Current state or YMCA approved first aid certification or acquired within 3 months of hire.One or more years of college education in health promotion, public health, or a related field. Other applicable education, training, and experience, which provide the knowledge, abilities, and skills necessary to perform effectively in the position will be considered. Two or more years' experience working in health promotion, public health, or community outreach or similar field. Community Health Worker (CHW) certification Knowledge of and previous experience with diverse populations (language, culture, race, physical ability, sexual orientation, etc.). Ability to speak any language in addition to English Knowledge of community resources available to public within Pierce and/or Kitsap Counties Previous experience interacting with the healthcare system. Complete online Child Abuse Prevention training on first day; YMCA Orientation to Healthy Living Training and Motivational Interviewing training and/or YMCA Listen First training within 3 months of hire.
