BSN and 2-years of experience with Neurology. This position entails following a patient that has had a stroke or seizure 90 days post discharge. Follow up with the patient if they go to a SNF/LTAC make specialist or PCP follow up appointments medication management home healthcare and will assist with getting the patient to a clinic if there are noted concerns.
