Growing home health & hospice agency seeking dynamic intake coordinator who will provide general office administrative duties. If you are someone who is not afraid of a fast paced environment and willing to help others when the need arises. This position includes, but is not limited to, answering phones, managing fax communications, entering patient referrals into the EMR system, assisting with coordination of services, creating, modifying and copying forms as needed, filing, etc. You will also handle and distribute Medicare paperwork, tracking physician orders, assembling admission packets, assisting with quality assurance activities and regulatory compliance. INTAKE COORDINATOR QUALIFICATIONS:

  • Possess a valid Driver’s License and reliable transportation.
  • Minimum of 1 year of Home Health or Healthcare experience preferred.
  • Proficient in Microsoft Office Suite applications Word, Excel, etc.
