Job Description:

*Lead Care Manager LCSW at our Health Center, located in Santa Monica, CA. The position available is for Full Time, 12:00pm-8:30pm shift.

Required qualifications:

*Master's Degree in Social Work from an accredited program.

*Upon hire: California Clinical Social Worker License

*2 years of experience in healthcare related field (Acute, Ambulatory, or Post-Acute)

*2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.

Candidate Details:

*2+ to 5 years experience

*Seniority Level - Mid-Senior

*Management Experience Required - Yes

*Minimum Education - Master's Degree

*Willingness to Travel - Always
