Job Title: Medical Social Worker (MSW)

Location: San Jose, California

Job Type: Contract

Duration: 13 Weeks

Shift: 8-hour shifts, 5 days per week

Position Overview:

Pride Health is seeking a compassionate and skilled Medical Social Worker (MSW) to join our team. This role is designed to provide psychosocial support to patients undergoing dialysis treatment and living with chronic kidney disease (CKD). The MSW will conduct comprehensive psychosocial assessments, develop care plans, and offer emotional support to patients and their families. This position is ideal for a social worker looking to make a tangible difference in the lives of individuals managing kidney disease.

Key Responsibilities:

• Conduct psychosocial assessments and evaluations of patients' emotional and social needs.

• Develop individualized care plans in collaboration with the interdisciplinary team (IDT).

• Provide supportive counseling, crisis intervention, and referrals for long-term care.

• Educate and empower patients and families about dialysis, CKD, and treatment adjustments.

• Participate in interdisciplinary team meetings, patient care conferences, and QAPI activities.

• Maintain accurate and compliant documentation of social work services (SPIN).

• Facilitate patient and family orientation to dialysis care, ensuring smooth transitions and adjustments.

• Collaborate with external providers and agencies as necessary for patient care.

• Lead or co-facilitate support groups or psychoeducation sessions for patients.


Qualifications:

Required Education : Master’s Degree in Social Work (MSW) from an accredited program.

Licensure : State Social Worker License (LSW) as required by state regulations.

Certification : BLS certification from the American Heart Association or American Red Cross.

Experience : One year of experience in a behavioral or healthcare-related setting preferred. Dialysis or CKD experience is a plus.

Skills : Strong communication, crisis management, and counseling skills; ability to work effectively within a multidisciplinary team.
