POSITION SUMMARY

The Behavioral Health Provider will provide behavioral health assessment, diagnosis, on-going therapy, and provide same day and crisis interventions to patients referred by High Plains Community Health Center (HPCHC) medical providers, self-referred, and community referred. Interagency and interdisciplinary care coordination and consultation as appropriate to promote positive patient outcomes. Provide community outreach to promote whole person, 360 care model of HPCHC.

PRODUCTIVITY AND EXPECTATIONS:

The Behavioral Health Provider is an essential component to the success of HPCHC. This position is expected to prioritize time and responsibilities in the following manner:


70%: Clinical Activities

  • Practice within an integrated, biopsychosocial, team based, patient centered model of care.
  • Provide behavioral health assessments, therapy, and brief interventions to patients.
  • Provide behavioral health services appropriate to the age and culturally specific needs of patients.
  • Provide crisis intervention as needed.
  • Provide clinical supervision of behavioral health students and/or license candidates providing care at the health center’s facilities as assigned by Director.
  • Engage patients, colleagues, and community to promote whole person care.
  • Collaborate with HPCHC medical staff to provide whole person care and advocate for understanding of the relationship between mental health issues and physical issues.
  • Perform the NADA 5-point protocol once certified.
  • Participate in all-staff meetings, department meetings, and other meetings as assigned.


15%: Documentation

  • Take biopsychosocial histories, developing treatment plans and writing reports.
  • Maintain proficiency in EHR and complete clinical documentation in a timely manner, defined as within 48 hrs. of clinical contact.


15%: Community Outreach

  • Provide community outreach as assigned to promote the services of HPCHC.
  • Occasional travel may be required including day-to-day local travel.


QUALIFICATIONS

  • Master’s degree in social work, counseling or psychology is required.
  • State of Colorado Behavioral Health Provider License is required (LPC, LAC, and or Candidate).
  • Eligibility to bill Medicaid, Medicare, and commercial payers.
  • Current Basic Life Support Certification (BLS) is required.
  • A minimum of two years of experience working with all ages, diverse and/or underserved populations is required.
  • A minimum of two years of experience working with individuals, groups, family, and conjoint treatment is required.
  • A minimum of two years of experience with brief and/or solution focused therapy as well as basic therapy techniques such as: cognitive behavioral therapy, mindfulness, or psychodynamic therapy is required.
  • Understanding of psychotropic medication uses and side effects in treatment of patients is required.
  • A minimum of two years of experience with using DMS-5 diagnostic criteria and methodology is required.
  • Knowledge and correlation of physiological problems and psychological function is required.
  • Certified or Licensed Additions Specialist preferred.
  • Experience with telehealth preferred.
  • Experience with providing supervision to students and/or candidates is preferred.
  • Spanish language skills are preferred.
  • A minimum of two years of experience working with advanced therapeutic techniques such as EMDR, DeTur, Trauma Processing, Emotion freedom technique, auricular acupuncture (acudetox), or hypnosis is preferred.
  • Demonstrated experience of participation or collaboration with community initiatives preferred.

  • Works both independently and as part of a team; establishes and maintains effective working relationships with health care team members and diverse patient populations.
  • Displays time management and organizational skills to consistently meet deadlines.
  • Performs other duties as required.
