Join our team as a contracted Social Worker for our LTC facility client in Stillwater, MN! A great opportunity to get into the world of travel social work, within a great facility.

Up to $2,086 weekly travel pay

Local Rate: $45-$47 hourly

Shift: M-F 8 am-5 pm

Contract Length: 02/03-05/05

Duties:

• Provides supportive services/counseling on healthcare and home care programs and services.
• Serves as a member of the interdisciplinary team in assisting with social, emotional and economic concerns of patients/clients/residents and families/caregivers, thus enabling them to achieve or maintain an optimal level of functioning by coordinating and planning programs.
• Coordinates healthcare programs among patients/clients/residents, families/caregivers and psychosocial and healthcare teams/communities.
• Demonstrates knowledge of human behavior and developmental stages.
• Provides referrals, current information, and/or education regarding programs and services available.

Requirements:

• Active Minnesota LSW license.
• BLS certification within 6 weeks of employment
• Long Term Care experience-Admissions, Social Work, Care Conferences, Documenting, Liaison with County, Social needs of Residents, Discharge Planning
